Ask whether the clinic accepts online forms, phone intake or both, and when the information was last updated

I found it useful to keep a dated list of every clinic or program contacted, including whether the answer was accepting, waitlist or not accepting.

I saved confirmation numbers and copies of forms so I could follow up without starting over.
